All camp forms must be mailed to:

Please fill out application form and be sure to include your child's hockey sweater size.

Method of Payment:

Don't send cash! Pay by check payable to HARTLEY HOCKEY INC.

Each applicant must be accompanied by 50% or more of each player's total registration fee.
Balance of payment is due before June 1st, 2012

Cancellations:

Any cancellations by a participant before June 1, 2012, all deposits/funds will be refunded less $75.00.

After June 1, $150.00 administrative fees will be taken off deposits/funds.

After July 1, no refunds will be provided (substitutes will be accepted).
